Providing a comprehensive and integrated range of support services for frail older people and others with a disability and their carers. This program is to assist older people and people with disabilities to be more independent in their own home, thereby preventing their inappropriate admission to residential care and enhancing their quality of life.

It also assists in supporting and enhancing the family and carers key role in the provision of support. Care recipients must be aged 65 years and over and living in the Grampians region.
